Decision	Type picklist Properties Create, Filter, Group, Nillable, Sort, Update Description Specifies the decision of the evaluation. Picklist values for this field aren't predefined. Before you create a record, ensure that your Salesforce admin has defined picklist values according to your organization’s requirement.
EvaluatorId	Type reference Properties Create, Filter, Group, Nillable, Sort, Update Description The user or group who evaluated the application form. This field is a relationship field. Relationship Name Evaluator Refers To User
LastReferencedDate	Type dateTime Properties Filter, Nillable, Sort Description The timestamp for when the current user last viewed a record related to this record.
LastViewedDate	Type dateTime Properties Filter, Nillable, Sort Description The timestamp for when the current user last viewed this record. If this value is null, it’s possible that this record was referenced (LastReferencedDate) and not viewed.
Name	Type string Properties Autonumber, Defaulted on create, Filter, idLookup, Sort Description The name of the application form evaluation record.
