Create Code Sets for Health Care Metrics
Code sets define the metrics that providers monitor in patients, for example, blood sugar, cholesterol level, and amount of exercise. Define the code sets your providers use with their patients. -
Create Units of Measure for Health Care Metrics
Define units of measure to use with code sets for monitoring healthcare metrics. For example, to monitor a patient’s amount of exercise, create a unit of measure called Hours Per Week. -
Set Org-Level Care Metric Targets
Care metric targets represent health care goals. As an admin, you can set org-level care metric targets that care coordinators can use as a starting point when creating targets for specific patients. Setting org-level care metric targets is optional. -
Add Care Observations to the Patient Account Page
Display care observations as a related list on the patient account page. Care observations help care coordinators track patients’ real-time biometric data. -
Display Remote Monitoring Charts on the Patient Account Page
Charts that show biometric data and health metrics such as heart rate, blood pressure, or blood glucose levels help care coordinators track patient health.
