Home Health
Use Home Health to efficiently schedule and manage visits for your organization’s home
healthcare services. With this feature, your users can automate large parts of their operations,
such as scheduling recurring visits and managing field resource assignments.
-
Learn About Home Health
Get to know the different users of Home Health, the field-level operations involved in home visits, and visit scheduling and assignments. Learn about the patient eligibility verification and electronic visit verification workflows that can expedite and streamline your home visit operations. -
Learn About Home Health’s Integrations
Get to know how Home Health integrates with Utilization Management, Integrated Care Management, and Medication Management to enhance patient health outcomes and operational efficiencies. -
Home Health Data Model and Permissions
Home Health is primarily built over the Field Service data model. Here’s the set of objects, metadata types, and permissions that you get with Home Health. -
Enable Home Health
Before your users can start using Home Health, you have to enable the feature, enable Field Service, and install the Field Service managed package. -
Get Your Org Ready to Use Home Health
Before you get started with Home Health, you have to complete a few standard tasks to prepare your org for the feature. You have to create profiles for your users and manage permission set assignments. You also have to make some changes to certain objects and generate the default scheduling policy. -
Set Up Field Resources, Regions, and Times for Home Health
Your resources in the field are the bread and butter of your organization. Create Service Resource records to represent those users. Then, create Service Territory records to represent the regions they operate in. And finally, associate service resources to service territories using Service Territory Member. -
Define Service Report Templates for Home Health
Service Report Templates help your users quickly generate reports for the visits they complete in the form of a PDF. These reports can be shared with customers and supervisors to ensure that everyone’s on the same page about how the visit was performed and have the details of the visit on the record. You can define Service Report Templates for your org from the Service Report Templates page in Setup, and assign templates to the appropriate work types. -
Define Skills, Preferences, and Work Types for Home Health
Home Health assigns field resources to the right visits based on the skills required for each type of service and the preferences the patient has. To record all this data in our system, we use the Work Type and Skill objects, and other related objects. -
Define Work Plan Templates for Home Health Visits
Work Plan Templates add tasks to scheduled visits that the field resource has to complete before ending each visit. These tasks could either be simple checklist items, or they could launch flows that the field resource can use to capture data for your system. Work plan templates are set up for work types, and are used to generate work plans for the work orders created for those work types. -
The Home Health User Interface
Home Health comes with an app and three lightning components that your schedulers can use. And your field resources have access to Home Health on the Field Service mobile app. -
Handle Field Resource Absences in Home Health
Home Health gives you a record-trigger flow that updates scheduling conflicts when field resources assigned to visits take time off. Visits affected by a resource’s absence are updated with a new status, and their resource assignments are removed. -
Enhanced Scheduling and Optimization for Home Health
Because Home Health is built over Salesforce Field Service, it can use the Enhanced Scheduling and Optimization feature to optimize visit assignments for your org. The Enhanced Scheduling and Optimization jobs that you set up can be configured to run everyday and reassign visits for optimal field resource utilization. The goal of this optimization is to use point-to-point predictive routing so that your field resources spend more time in appointments and less time traveling between them. -
Integrate Home Health with Integrated Care Management
Help your users to create enhanced care plans, assign them to home visits, and coordinate care more efficiently for their patients. Care plans focus on the patient’s unique set of needs, preferences, and health problems. Coupling a care plan with the right home care services provides a patient-centered approach to care. -
Set Up Electronic Visit Verification
Help your care resources and schedulers to verify visits automatically or manually and save the visit start and end details accurately. -
Set Up a Briefcase for Home Health
Enable your clinicians in the field to access information seamlessly when their mobile app is offline. Use Briefcase Builder to create a briefcase, which is a set of rules and filters that select records for offline use. Connect the Field Service Mobile app with the briefcase to give your clinicians access to the most relevant records, even when there’s a network outage or spotty internet coverage. -
Artificial Intelligence for Home Health
Get the top three recommended care professionals for a patient and use this information to swiftly schedule the patient's appointment with the most appropriate care professional.
