Unified Health Scoring
Unified Health Scoring gives your users a holistic view of their patients’ health
profiles. Using this feature, you can define a unified health profile for your organization that
shows your users everything concerning their patients’ health. You can also configure contextual
actions that prompt your users to intervene based on a patient’s score. This multidimensional view
together with contextual prompts help your users plan their interventions better for improved
health outcomes, greater patient engagement, and reduced healthcare costs.
What You Get
Here’s what you get with Unified Health Scoring:
- Three metadata types
- Score Category: Defines the different categories and subcategories of a unified health profile.
- Score Range Classification: Defines and classifies the score ranges of a category or subcategory.
- Score Category Calculated Insight: Maps a score category or subcategory to it’s calculated insight in Data Cloud. Don’t worry, we’ll talk about how Data Cloud fits into the picture in a bit.
- Three standard objects
- Health Score: Stores the score information for patients.
- Health Score Action Log: Records the log of actions triggered from the Dynamic Actions for Unified Health Scoring component.
- Action Visibility: Assists in defining contextual visibility rules for actions in the Dynamic Actions for Unified Health Scoring component.
- Three lightning components
- Score Details for Unified Health Scoring: Displays the historical score trends for a patient’s unified health profile and it’s subcategories. This information is also displayed as graphs.
- Score List for Unified Health Scoring: Displays a list of patients with score changes in the last fifteen or thirty days and the degree of their score change.
- Dynamic Actions for Unified Health Scoring: Displays a set of contextual actions your users can take based on the visibility rules you define for them.
- Two connect APIs
- Score Synchronization API: Updates score information in Health Score with the latest information processed by calculated insights in Data Cloud.
- Synchronization Status API: Displays the status of a synchronization request.
Integration with Data Cloud
While Unified Health Scoring can render score information in an easy-to-interpret user interface, it can’t calculate that score information for you. That’s where Data Cloud comes in. You can export a patient’s health records to Data Cloud and use the Calculated Insights feature in Data Cloud to process those records and derive scores. You can then import those scores back into Health Score object in Health Cloud. This integration allows you to define custom logic to derive scores using the data crunching capabilities of CDP.
If you prefer not to use Data Cloud, you can always use custom solutions to derive health scores. But be sure to import those scores into the Health Score object.
