Integrate Data Cloud with Unified Health Scoring

While Unified Health Scoring can display a patient’s score information and historical score trends in a user-friendly interface, it doesn’t calculate those scores for you. Use Data Cloud to aggregate a patient’s related records and derive the required scores.
Available in: Enterprise and Unlimited Editions with Health Cloud and the Unified Health Scoring Add-On license

You can use Unified Health Score without using Data Cloud, but you have to provide a custom solution that derives scores and imports those scores into the Health Score object.

Note

Data Cloud uses data streams to pull information from Health Cloud and store that information in custom data model objects. You can then use Calculated Insights in Data Cloud to aggregate that information and derive scores. And finally, use the Score Synchronization API to retrieve those scores from Data Cloud and insert them into Health Cloud. But we’ll talk about that last part later.
