Create Predefined Tasks for Medication Therapy Management

Help your pharmacists to effortlessly recommend to-do list items to their patients during medication therapy reviews. You can set up predefined, patient-specific tasks for addressing healthcare issues that pharmacists can assign to patients.

This preconfiguration helps to save your pharmacist’s time and enforces care standards to improve healthcare outcomes. Also, with patients being active partners in their healthcare, there’s improved patient engagement, self-care, and satisfaction.

Make sure you enable the To-do Lists settings from the Medication Management and Medication Review Settings page. This setting enables the Care Plan, Care Plan Activity, Care Plan Template, and Action Plan Template Assignment objects to help you create predefined tasks.

To configure care plan templates: Create and Edit on Care Plan Template object
To configure action plan templates: Create and Edit on Action Plan Template object
To configure action plan template assignments: Create and Edit on Action Plan Template Assignment object

Patient-specific tasks can cover medical, lifestyle, behavioral, or other aspects. Just be sure they all aim to resolve the patient’s problem and help achieve health goals.

  1. For each issue (health or non-health) that patients face, configure a care plan template, publish it, and save your changes. For example, create one for High Blood Pressure and one for Diabetes.
  2. Create a corresponding action plan template.
    1. Set the Action Plan Type as Industries and Target Object as Care Plan. Also, select Let users add items to action plans.
    2. Define issue-specific tasks and add them to the action plan template.
    3. Publish the action plan template and save your changes.
  3. Create an action plan template assignment to associate the care plan template and action plan template.
    1. Select the care plan template that you created from the Associated Object list.
    2. Select the action plan template that you created from the Action Plan Template Version list.
    3. Save your changes.

    If you don’t publish the care plan template and action plan template, your users can’t use any of the related predefined tasks.

    Note
