Create Care Programs for Medication Therapy Review

Use Health Cloud’s Care Program feature to create a care program for medication therapy reviews for the corresponding year. Pharmacists can conduct comprehensive and targeted medication reviews for patients who meet eligibility criteria and are enrolled in the care program.

Available in: Enterprise and Unlimited Editions with Health Cloud and the Medication Management Add-On license

User Permissions Needed
To create care program and care program enrollee records: Create and Edit on Care Program and Care Program Enrollee objects
  1. From the App Launcher, find and select Care Programs.
  2. Click New.
  3. Enter the name, start date, and other important details about the medication therapy management care program.
  4. Save your changes.

Next, go to Care Program Enrollee and create records for patients who are eligible and enrolled in the care program.

If eligibility rules are set up for the care program using Data Processing Engine or Business Rules Engine, patients are auto-enrolled and care program enrollee records are auto-created.
