jCreate a Care Plan Template Problem
Care plan template problems are health diagnoses, such as high blood pressure or
diabetes, that can be reused for different patients. Care coordinators can use care plan template
problems to provide standardized care across similar problems and patients. You can create a care
plan template problem to use in your care plan template.
Create a template problem for a specific care plan template, directly from that template’s related list. You can also create a custom Care Plan Template Problems tab and add the tab to the Health Cloud custom apps. You can include any additional fields you need to the Care Plan Template Problem object’s page layout.

If you’re creating a template problem for a specific template directly, go to that record’s
related list. And then click New for Care Plan Template Problems.
If you’ve created a custom Care Plan Template Problems tab, go to App Launcher, select the Care Plan Template Problems tab, and click New.

Enter the following:
Field Description Care Plan Problem Name Name of the problem for the care plan template. Care Plan Template Name of the care plan template this problem is related to. Description Provide a brief description of the problem that is being addressed. Active Select to activate this care plan template problem and make it available to apply to a patient or member. Sort Order Indicates the vertical order in which this problem appears on the care plan template. Priority The priority of the care plan problem such as low, normal, or high. Category Indicates the category of ailments the template problem belongs to. - Click Save. The new template problem opens in a new tab.
