Care coordinators can use care plan template problems to provide standardized care across similar problems and patients. You can create a care plan template problem to use in your care plan template.

Care plan template goals are common health targets that care coordinators can reuse in different care plans. Care coordinators can add predefined goals to a patient’s care plan, saving time and effort.
