Care Gaps | Health Cloud Administration

When you enable and configure care gaps in Integrated Care Management, care managers can evaluate clinical measures, identify gaps in patient care, and create care plans from care gaps. The care gap FlexCard in the patient record page gives users a comprehensive view of a patient’s care gaps. This FlexCard also has a New Care Gap button that launches the OmniScript flow for evaluating a clinical measure and creating a care gap.

Enable Care Gaps
Enable care gaps in Integrated Care Management so that your users can identify gaps in patient care. When you enable care gaps, care managers can access the HealthCloudCareManagementCareGapManager FlexCard, the OmniScript flow for evaluating a measure and creating a care gap, and the option to create care plans from care gaps.
Add the Care Gaps FlexCard to the Patient Record Page
Enable care managers to identify and update care gaps by adding the HealthCloudCareManagementCareGapManager FlexCard to the patient (person account) record page.
Enable Users to Create Care Gaps by Evaluating a Measure
Create a clinical measure and implement a trigger-based logic to evaluate the measure so care managers can create a care gap using the measure evaluation flow.
