Create an Action Plan Template

Action Plan Templates instantiate to action plans and their interventions when templates that use them are used to create care plans. You can record commonly used information in these definitions, such as the name, priority, duration, and so on. This information is copied over to Action Plan, Action Plan Item, and Task records when they’re instantiated.

To create an intervention Create access for Task
  1. From the App Launcher, find and select Action Plan Templates.
  2. Enter a name and description for the action plan template.
    These values are copied over as the name and description of action plans instantiated from this template.
  3. If you want to let users add more interventions to action plans created from this template, select Let users add items to action plans .
    Selecting this option helps care managers add interventions to an existing care plan and its action plan.
  4. Select Industries as the action plan type.
  5. Select a target object based on the type of interventions you’re defining in the action plan.
    • For interventions for goals, select Goal Assignment.
    • For interventions for conditions, select Health Condition.
    • For interventions for social determinants of health, select Care Barrier.
    • For interventions for care plans, select Care Plan.
  6. Click Save.
  7. Now that you’ve created the base for an action plan template, let’s add some interventions to it.
    1. Click New Task.
    2. Enter a subject.
      This value is the name of the intervention that is instantiated in the corresponding Task record.
    3. Set the priority to High, Normal, or Low.
    4. Enter the number of days that the intervention is meant to be completed in.
      The due date for the intervention is calculated by adding this value to the date and time when the action plan is instantiated.
    5. If the intervention you’re defining is a mandatory action, select Required.
      If this intervention is meant to be a prerequisite for another intervention, then it has to be marked as required.
    6. If the intervention you’re defining has prerequisite interventions, select them in the Prerequisites section.
    7. In the Task assignment section, select Action Plan Creator.
      Selecting this options ensures that all the interventions generated from this action plan are by default assigned to the care manager who created the action plan.
    8. If necessary, set an appropriate time in the Reminder section.
    9. Save your work.
  8. After you add all the necessary interventions to the action plan template, click Publish Template and click Publish.
