Create a Problem Definition
Problem definitions instantiate to problems when templates that use them are used to
create care plans. You can record commonly used information in these definitions, such as ICD
codes, names, descriptions, and so on. This information is copied over to Health Condition records
when they’re instantiated.

- From the App Launcher, find and select Problem Definitions.
- Click New.
- Enter a name and description.
- Select a priority for this problem.

Select a code set or code set bundle representing an ICD code in the
Code field.
If your problem is a condition, select the code that represents the condition. If your problem is a social determinant of health, select the code that represents the type of care barrier.
- Select Problem List Item in the Category field.

Select Health Condition or Barrier in the
Usage Type field.
If the problem is a condition, select Health Condition. If the problem is a social determinant of health, select Barrier.
- If your problem is a social determinant of health, select a barrier type.
- Select Active.
- Save your problem definition.
