Add Problems with Goals to a Care Plan Template

You can add problems with goals to a care plan template by creating problem-goal junction records. A problem-goal junction associates a parent problem definition with its child goal definition. These junction records are then referenced in Care Plan Template Problem records.

To create a problem template Create access for Care Plan Template Problem
To edit a problem template Edit access for Care Plan Template Problem
To create a problem goal definition Create access for Problem Goal Definition
To edit a problem goal definition Edit access for Problem Goal Definition
  1. Create a problem-goal junction.
    1. From the App Launcher, find and select Problem Goal Definitions.
    2. Click New.
    3. In the Problem Definition field, select the parent problem definition from your PGI library.
    4. In the Goal Definition field, select the child goal definition from your PGI library.
    5. Save your work.
  2. Create a Care Plan Template Problem record.
    1. From the App Launcher, find and select Care Plan Template Problems.
    2. Click New.
    3. In the Care Plan Template field, select the care plan template that you want to add the problem and its goal to.
    4. In the Problem Definition field, set the reference to Problem Goal Definition and select the problem-goal junction you created.
    5. Enter a sequence number.
      This value is used to determine how the problems in your care plan are sorted.
    6. Set the priority as High, Medium, or Low.
    7. Save your template.
